This presentation will provide participants with the current evidence-based best practice for creating, revising, and communicating effective, individualized care plans. It will provide a brief overview then delve into specific requirements for care plan development related to current expectations for subjects including but not limited to: behavior care plans; pain management care plans; non-pharmacological interventions care plans; new order care plans; psychoactive medication care plans; revision of care plans.

Objectives:
Participants will demonstrate understanding of the purpose of effective care plans and service plans
Participants will demonstrate understanding of the State & Federal regulatory requirements of effective care plans
Participants will demonstrate understanding examples of what should be included in specific care plans for behavior; pain management; medication management; ordered treatments; non-pharmacological interventions; new orders; psychoactive medications
Participants will demonstrate understanding of how/when & who should revise care plans & service plans
Participants will demonstrate understanding of how to effectively communicate care plan information to residents and care partners
